Meet the specialist team providing compassionate Hospice at Home support for patients and families.
Our Hospice at Home Team provides compassionate palliative and end-of-life support in partnership with St Luke’s Hospice. The team works alongside patients, families and other professionals to deliver coordinated care in familiar surroundings.

I have worked in care for around 15 years, gaining experience across a wide range of settings, including nannying, care for older people, community care, care homes and reablement. I enjoy all aspects of care work, particularly building relationships and interacting with the people I support.
I joined the Hospice at Home team in June this year and am now a Healthcare Facilitator. My role involves coordinating rotas for the care team, supporting patients and families with enquiries, and helping to ensure they receive the best possible care. I also occasionally visit patients in their homes to support their needs, which is a part of my role I particularly enjoy.
